Job Summary

Assist individuals with their physical, economic, vocational, recreational, social, emotional and daily life skills development. The Community Support Worker assists individuals to achieve the highest quality of life possible. Promotes and upholds the rights of individuals we serve and honours their choices in accordance with the policies and procedures of the agency.

Key Duties and Responsibilities

  1. Participates in assessment, goal setting and program planning such as Individual Service Plans for individuals living in the community.  Documents and implements the plan.  Provides input into the evaluation of the program.
  2. Evaluate individual’s needs and develop (with support team) short term plans to meet such needs with active participation of the individual, their families and/or residential support
  3. Identifies social, economic, recreational, physical, vocational and educational services in the community that will meet the individual’s needs; provide support, guidance and instruction to facilitate participation.  Maintains liaison with other agencies, the individuals’ family/residential support, professionals, government personnel and members of the community.
  4. Assist individuals to function more independently in their own home and in the community in accordance with established procedures and protocols.  Assists individuals with daily life skills, social skills and /or behaviour management.  Teaches and assists individuals with activities such as grooming, basic cooking, money management, shopping, household safety and pet care.  Ensures the safety of the individuals.
  5. Recognizes, analyzes and responds to potential emergency situations such as individual’s aggressive behaviour or medical emergency to ensure no harm comes to the individual, employees and/or the public.  Reports concerns to the program manager.
  6. Performs delegated tasks as directed by professionals, such as administering medication to individuals in accordance to doctor’s instruction, within established policies and individual care plans.
  7. Reviews and evaluates individual’s progress and makes adjustments to programs as required.  Provides feedback and support to individuals and/or their families.
  8. Accompanies and/or transports individuals to activities such as appointments, shopping or leisure activities.
  9. Provides input into the agency’s continuous quality improvement process as required.
  10. Provides written and/or verbal reports regarding individual’s daily activities and progress. Maintains reports and records such as logs and accreditation outcomes data collection reports.  Ensures that all required documentation is complete and accurate in accordance with established policy.
  11. Uses sound judgement; carries out job duties in a professional manner and is guided at all times by the agency’s Code of Ethics.
  12. Performs other related duties as required.

Qualifications: Education, Training and Experience

  1. One year certificate or equivalent in a related human / social service field with emphasis on community living services.
  2. One year recent related experience in community living. Exhibits a healthy attitude towards normalization principles and is aware of their position as a positive role model.
  3.  Demonstrated ability in written, oral and interpersonal communication skills in English (at a minimum Grade 10 level).
  4.  Have knowledge of and experience in Behaviour Management.  Demonstrated ability to interact effectively with the individuals.
  5. Demonstrated ability to work independently within established agency policies and procedures.
  6. Able to meet applicable Ministry legislation and regulations (e.g. Community Care and Assisted Living Act and applicable regulations) and agency policies.

Employment Requirements

  1. Valid Class 4 Restricted BC driver’s licence and a drivers abstract showing fewer than 4 tickets that carry penalty points in previous 2 years and no motor vehicle related Criminal Code of Canada conviction in the previous 3 years
  2. Vehicle suitable for transporting 4 or more people, with business use insurance coverage
  3. Valid First Aid and CPR certificates
  4. Valid Food Safe Certificate as required by program manager or designate
  5.  Current and negative Mantoux (TB) test
  6. Valid, current and clear Criminal Record Check completed through the CRIMINAL RECORDS REVIEW PROGRAM of the Ministry of Public Safety and Solicitor General, Government of BC.  There is a $20.00 cost for this.
  7. Good general health and physically able to perform the duties of the job (Doctor’s certification required 14 days prior to commencement of employment)
  8. Must sign the agency’s Statement of Confidentiality and Acknowledgement of Agency Abuse Policy
